What it is
PT-141 is the development name for bremelanotide, a small cyclic peptide that activates melanocortin receptors in the brain. It was developed from melanotan II, the tanning peptide whose early human studies showed erections and arousal as a side effect. Bremelanotide kept the sexual effect and dropped the tanning goal.
It is one of the few compounds on this site with a full regulatory approval. The US FDA approved it in June 2019 as Vyleesi, a 1.75 mg autoinjector for premenopausal women with acquired, generalized hypoactive sexual desire disorder (HSDD). HSDD here means low desire that causes marked distress and is not explained by another medical or psychiatric condition, relationship problems or medication.
How it works
Bremelanotide is a melanocortin receptor agonist, with most of its relevant activity at the MC4 receptor and some at MC3. These receptors sit in hypothalamic circuits that handle appetite, energy balance and sexual motivation. The FDA label is candid that the exact way it improves desire in women "is unknown".
That central mechanism is the main difference from erectile dysfunction drugs. Sildenafil and tadalafil relax blood vessels in the penis and do nothing for desire. Bremelanotide acts upstream, in the brain.
What the research found
Human trials in women. The approval rests on the two identical RECONNECT phase 3 trials (Kingsberg and colleagues, 2019). 1,267 premenopausal women with HSDD were randomised to bremelanotide 1.75 mg or placebo, injected as needed, over 24 weeks. Compared with placebo, the bremelanotide groups had statistically significant increases in sexual desire scores and reductions in distress about low desire in both trials. The absolute changes on the questionnaires were modest, which is a fair summary of how the drug performs: it moves the needle for some women rather than transforming desire across the board.
Human trials in men. PT-141 was also investigated in men with erectile dysfunction. That work did not lead to an approval, and the current label states the drug is not indicated for men.
Side effects reported in studies

Nausea is the defining problem: 13% of treated women needed anti-nausea medicine and 8% stopped because of it. Blood pressure rises transiently after each dose, by up to 6 mmHg systolic and 3 mmHg diastolic, peaking 2 to 4 hours after injection, with heart rate dropping by up to 5 beats per minute. For that reason the label lists uncontrolled high blood pressure and known cardiovascular disease as contraindications. Focal darkening of the skin occurred in 1% of women on up to 8 doses a month, more often in people with darker skin, and did not always resolve. That last point is relevant for Filipino skin types.

The key distinction from melanotan II is selectivity. Melanotan II also hits MC1R, the pigment receptor, which is why it tans and why it carries mole and melanoma concerns. Bremelanotide was built to reduce that, although the label still records skin darkening.
Status in the Philippines
No bremelanotide product is registered with FDA Philippines, so Vyleesi cannot be bought here as a registered medicine. PT-141 is not a controlled substance under RA 9165. Locally it circulates as a research peptide, usually as lyophilised powder in 10 mg vials. As an approved medicine elsewhere, prescribing belongs with a licensed Philippine physician, who can also check blood pressure and cardiovascular history first. Storage
Lyophilised powder keeps best refrigerated and away from light. Once reconstituted with bacteriostatic water, keep it at 2 to 8°C.
